Abstract

Background/aim: The objective of this study was to evaluate the validity and reliability of the Turkish version of the NOSE-Perf scale, developed to assess symptom severity and quality of life in patients with nasal septal perforation (NSP), and to compare the symptom profile of patients with NSP with those of the nasal obstruction and healthy control groups.

Materials and methods: To evaluate the temporal stability of the NOSE-Perf scale, a test–retest assessment was performed in the NSP group at a 1-month interval. Internal consistency was assessed using Cronbach’s alpha coefficient, while discriminative validity was evaluated using receiver operating characteristic (ROC) analysis.

Results: The Turkish version of the NOSE-Perf scale demonstrated acceptable internal consistency (Cronbach’s alpha = 0.759) and excellent test–retest reliability (ICC = 0.978). While the conventional NOSE scale did not differentiate between the NSP and nasal obstruction groups (p = 0.902), NOSE-Perf scores differed significantly among the three groups overall (p < 0.001) and were significantly higher in the NSP group than in the nasal obstruction group (p = 0.017). In the ROC analysis, NOSE-Perf showed acceptable discriminative ability to distinguish NSP from nasal obstruction (AUC = 0.694), whereas the NOSE scale showed approximately chance-level discrimination (AUC = 0.495). When healthy controls were also included in the comparison group, discriminative ability increased further (NOSE-Perf: AUC = 0.845; NOSE: AUC = 0.735).

Conclusion: The Turkish version of the NOSE-Perf scale is a valid and reliable instrument for assessing symptom burden in patients with NSP. By capturing perforation-specific symptoms—notably crusting, whistling, and epistaxis—the scale demonstrates an advantage over the conventional NOSE scale in distinguishing patients with NSP from those with nasal obstruction.
