Abstract
Background /aim: to assess the impact of microvascular Doppler ultrasonography (MDU) used intraoperatively during children's microsurgical subinguinal varicocele correction.Materials and methods: Nineteen patients who received intraoperative MDU during subinguinal microsurgical varicocelectomy between 2021 and 2023 were included in this study. The patients' age, varicocele side, clinical examinations, preoperative US results, intraoperative findings, spermatic artery counts and findings regarding the use of MDU, postoperative complications, and results were evaluated.Results: All varicoceles were on the left side and the average age was 15.2 years. The indications of varococelectomy were testicular hypotrophy(n=10) and scrotal pain or fullness(n=9). Under the surgical microscope, testicular artery pulsation was only detected in 5 patients, while using the MDU, arterial pulsation was detected in all cases. 16 had a single testicular artery 3 had two arteries, and identified. Additionally, in a case where a spermatic vein was suspected, it was not ligated due to the pulsation detected in an arterial pattern using MDU. 2-3 lymphatic channels were isolated and preserved and an average of 7.5 vessels were ligated, and 5 external spermatic veins were identified and ligated. There were no complications, and six of the individuals with testicular hypotrophy showed signs of the catch-up growth phenomena.Conclusion: The use of MDU during subinguinal microsurgical varicocelectomy in children not only increases the success rates but also minimizes complications such as hydrocele and recurrence.
